Longitudinal gastric resection
Indications for gastroplasty
The main indication is obesity above the second degree, when the body mass index (BMI) exceeds 35 kg/m2. Longitudinal gastric resection also helps to improve health in patients with concomitant diseases associated with obesity (type II diabetes mellitus, osteoarthritis, gastroesophageal reflux disease and hypertension). Unlike operations for cancer or peptic ulcer, gastric resection allows you to preserve all important functions of the stomach.
Advantages of longitudinal gastric resection in EMC

Longitudinal gastric resection surgery for weight loss
Longitudinal gastric resection is performed by laparoscopic or robotic methods.Laparoscopy
Laparoscopic longitudinal gastric resection is the surgical removal of a part of the stomach through punctures in the abdominal cavity. A "sleeve" is formed from the remaining part, which fully retains its functions.Robotic
The interaction of the robot and the surgeon ensures high-precision work on the tissues, so they are less injured, the risk of bleeding decreases, and their perfect connection occurs.Longitudinal or sleeve gastric resection is an operation that is used to treat overweight. This surgical procedure significantly limits the volume of the stomach and promotes hormonal changes to reduce hunger. At the EMC Clinic, longitudinal gastric resection is performed by world-class, highly qualified surgeons according to the most up-to-date international protocols.
What is sleeve gastric resection
Longitudinal resection refers to bariatric operations, that is, focused on combating obesity. During the operation, the doctor removes 80% of the stomach tissue, forming a sleeve-shaped organ. This removes the part of the stomach where the hunger hormone ghrelin is released. The patient gets saturated faster, eats less and, as a result, loses weight. It is important that, unlike radical operations used in the treatment of stomach ulcers or neoplasms, longitudinal resection completely preserves the functionality of the organ.
At the EMC clinic, the operation is performed using a robot-assisted method with laparoscopic access, that is, through miniature incisions, in a closed manner under video supervision. Due to this, the risk of complications is reduced, blood loss is reduced, and there are no noticeable scars or scars. Bariatric surgery using the Da Vinci robot allows you to avoid damage to even miniature vessels, to perform all manipulations as accurately and safely as possible for the patient.
Compared with other surgical methods of weight correction, sleeve resection provides the patient with important advantages:
- A multidirectional approach. The operation not only reduces the physical capacity of the stomach, but also reduces the patient's feeling of hunger.
- Long-lasting effect. If the patient adheres to proper nutrition, the lost pounds will not return until the end of life.
- There are no foreign bodies. Unlike gastric balloon surgery and other operations, sleeve gastroplasty does not install any foreign structures in the patient's gastrointestinal tract that will require further monitoring or create a risk of complications.
- The work of the organ is not disrupted. During the operation, the surgeon does not affect the gastric sphincters and areas responsible for the absorption of vitamins and trace elements. After sleeve resection, the patient is recommended to take dietary supplements for a year to maintain the body. After gastric bypass surgery, additional micro- and macronutrients are prescribed for life.
This method of bariatric surgery places special demands on the surgeon's professionalism and the characteristics of the equipment used.
Indications and contraindications for sleeve gastroplasty
Although sleeve resection has a good effect for weight loss, the operation is a complex surgical procedure and can cause complications, so it is performed only for medical reasons. These include:
- Third degree obesity (body mass index above 40).
- Second–degree obesity (BMI 35-40) in combination with at least one overweight disease (type 2 diabetes mellitus, hypertension, osteoarthritis, gastroesophageal reflux disease, severe obstructive sleep apnea).
- The lack of effect of conservative methods of weight loss in overweight.
There are few contraindications for surgery. Absolute contraindications include intolerance to medications for general anesthesia, serious psychiatric illnesses, and decompensated coagulopathy. Among the relative contraindications are Barrett's syndrome and severe gastroesophageal reflux disease.
Preparation for longitudinal gastric resection
When preparing for surgery, the patient is prescribed a special diet. This is necessary to lose some of the excess weight (optimally up to 10%). Following a diet reduces the risk of complications in the postoperative period, helps the patient get used to the lifestyle that will need to be followed after surgery, and also reduces fatty deposits near the liver that make it difficult to access the stomach. If the patient cannot get rid of excess weight on his own, an individual program is being developed for him with the participation of nutritionists, endocrinologists, psychologists and therapists.
Next, a thorough examination of the patient is performed to determine how much of the stomach can be safely removed. Preoperative preparation of the patient at the EMC clinic includes examinations by an anesthesiologist, cardiologist and therapist, ECG, ultrasound of the heart and vessels of the lower extremities, гастроскопию. This minimizes the risk of adverse events and complications after surgery.
At the EMC clinic, all diagnostic procedures can be performed on an outpatient basis, in one day, without the need to go to the hospital.
In addition, in preparation for the operation, the patient will have to take several blood tests, undergo a gastroscopy and other instrumental examinations according to indications.
2-3 days before the operation, the patient is shown a liquid diet — it is necessary to give up solid food and consume unpainted liquid products (broth, mashed soups, water). 12 hours before surgery, it is necessary to completely abandon food and drink.
How the operation works
The operation is performed under general anesthesia. After the start of anesthesia, a calibration probe is inserted through the oral cavity, which determines the permissible amount of intervention for gastric resection. Optical and microsurgical instruments are inserted into the abdominal cavity through instrumental punctures. By controlling the robotic instruments from his console, the surgeon extracts a tube from the stomach tissue around the calibration probe, shaped and sized like a banana. The remaining tissues are removed through one of the punctures in the abdominal wall. After the manipulations are completed, stitches are applied, the patient is transferred first to the postoperative ward, and after stabilization to a regular ward.
Advantages of gastric sleeve plastic surgery at the EMC clinic
- Highly qualified surgeons. We employ leading specialists in the field of bariatric surgery with many years of experience.
- The most modern equipment. Our doctors use advanced equipment, including the Da Vinci robot, the best Johnson & Johnson suturing machines, and the Harmonic ultrasonic scalpel for ultra-precise tissue extraction.
- Minimal risk of complications in the early and late postoperative period. We use special techniques to prevent undesirable events: bleeding, failure of the staple suture, hair loss due to a sharp weight loss, gallstones, etc.
- Convenience for overweight patients. Our wards and operating rooms have special equipment to make all patients feel comfortable.
