IMSI
IMSI (injection of morphologically selected sperm into the cytoplasm of an egg) is a new technology for assessing sperm quality, which is used during IVF instead of ICSI. The purpose of the technique is to select spermatozoa with the most favorable potential. The procedure consists in injecting a sperm cell directly into the cytoplasm of the egg, selected using morphological analysis of sperm.
IMSI is used to treat infertility in couples where it was caused by a male factor, and is indicated for severe teratozoospermia (morphological abnormality of spermatozoa). The procedure is also recommended for couples who have gone through several unsuccessful ICSI attempts.
IMSI is considered possible after a study of the morphology of motile spermatozoa, during which they are divided into four classes. Spermatozoa of only the first and second class are suitable for the procedure.: Positive morphological characteristics indicate a high probability of absence of DNA fragmentation and chromosomal
abnormalities.
Sperm selection for ICSI is carried out according to morphological criteria and takes much longer than the traditional ICSI procedure, but it can significantly increase the chances of couples with repeated unsuccessful ICSI attempts and with severe teratozoospermia.
The advantages of the technique also include a low probability of birth defects, miscarriages and pregnancy. Before performing an IMSI, the structure of spermatozoa is studied in detail under a microscope with a special lens and the image is displayed on a large screen. The magnification with IMSI is up to 6,000 times (600 — optical and 10 — digital), while with ICSI it is only up to 400. With the help of IMSI, it is possible to accurately see the structure of the sperm, identify or exclude abnormalities that can lead to miscarriages or problems with conception.
The IMSI method is used according to indications. In the absence of indications (severe teratozoospermia, repeated unsuccessful attempts at IVF ICSI), it does not provide statistically significant advantages over standard ICSI. If these indications are present, IMSI significantly increases the likelihood of a successful treatment outcome.
Couples who decide to use this method for the treatment of infertility should take into account that the issue of the IMSI procedure is decided individually and is accepted only voluntarily and after consultation with the attending physician.
