CT evaluation of coronary calcification in EMC

CT of coronary calcification in Moscow
According to the World Health Organization, the main causes of death of people under the age of 70, except for infectious diseases, are heart failure, cancerlung diseases and diabetes mellitus. One of the proven predictors of risk for coronary artery disease is the presence of an increased amount of coronary calcification.
Coronary calcification is one of the manifestations of atherosclerosis, when calcified plaques are deposited on the inner walls of the arteries feeding the heart . Coronary calcification is the final element of a series of changes in atherosclerotic plaques, which are formed gradually from "soft" plaques and represent irreversible changes. Plaques attached to the walls of the artery gradually narrow the lumen of the vessel through which blood flows. Large plaques create hemodynamic difficulties for the flow of blood and may even completely block the lumen of the vessel. A CT scan without intravenous contrast can only detect coronary calcification without the "soft" atherosclerotic plaques covering it. Usually, studies to determine coronary calcium are performed with ECG synchronization.

CT assessment of coronary calcium:

  • completed in 10 minutes;
  • is a non-invasive study;
  • it is performed in a supine position;
  • during its implementation, a minimum dose of radiation is used.

Contraindications

  • children's age,
  • pregnancy;
  • The limitation may be a high heart rate.
Assessment of the degree of atherosclerotic plaque is important for assessing cardiovascular risks. The amount of coronary calcium is estimated in Agatstone units. Usually, the level of coronary calcium increases with age. The complete absence of coronary calcium is considered the norm, as well as carriers of coronary calcium from 0-10 according to the Agatston index have a conditional insignificant risk. The assessment of coronary calcium levels is divided into 4 categories: 0 (none), 1-99 (moderate), 100-399 (moderate), and 400 or more (severe coronary calcification). Another widely used approach is to calculate the percentage by age and gender. The calcium index is a predictive assessment of the risk of cardiovascular events. The absence of coronary calcification indicates a very low risk of cardiovascular disease. According to the latest recommendations, calcium counting should be considered in asymptomatic individuals at risk.

In what cases should coronary calcium be assessed?

  • Arterial hypertension
  • Overweight
  • Smoking
  • Decreased physical activity
  • Age over 40 years
  • Diabetes mellitus
  • High coefficient of atherogenicity
  • Cardiovascular events in direct relatives
Special low-dose coronary calcium assessment studies are conducted at the EMC.
