Intravesical BCG immunotherapy for bladder cancer
Intravesical BCG immunotherapy is the most effective method of intravesical treatment of superficial bladder cancer. It reduces the recurrence rate and prevents the progression of malignant tumors. For bladder cancer, BCG therapy is significantly superior to chemotherapy and reduces the recurrence rate by an average of 40%, while chemotherapy reduces only by 14%.
The BCG vaccine is a suspension of weakened viable Calmette-Guerin bacilli. It was first used in 1921 to vaccinate people against tuberculosis. Further studies have shown that tuberculosis patients are less likely to develop malignant tumors after vaccination. This served as the basis for studying the effectiveness of this treatment method at the bladder level, where Calmette-Guerin bacilli, multiplying intracellularly, stimulate local immunity.
BCG therapy has been used for many years both abroad and in Russia, in particular in the EMC Urological Clinic. Many patients do not notice any side effects after the procedure, but rare cases have been described where BCG therapy could provoke cystitis or prostatitis.
Indications for the use of BCG therapy:

Bladder cancer

Tumor (including benign) of the bladder

Non-invasive (superficial) cancer
Contraindications to the use of BCG therapy:

Tuberculosis, including in the anamnesis

Hypersensitivity to the components of the drug

History of radiation therapy of the bladder

Congenital or acquired immunodeficiency (AIDS, leukemia, lymphoma)

Conducting immunosuppressive therapy
-
Severe concomitant diseases in the decompensation stage
-
Pregnancy and breastfeeding
Treatment regimen:
The course of treatment and the scheme are prescribed individually by an oncourologist according to the results of a histological examination. Usually, the standard induction course includes 6-8 weekly procedures. Approximately 2 hours after vaccination, the drug is concentrated in the bladder.
