In adult patients, hypospadias is most often formed due to unsuccessful surgical interventions. Such patients turn to a urologist with fistulas, rough skin scars that deform the penis, persistent curvature and dissatisfaction with the quality of intimate life associated with these problems.
Correction of such defects is always individual. Often, several operations are required to correct defects, including restoring the outflow of urine.
Stage 1 is the straightening of the penis, excision of rough scars that deform it, excision of problematic areas of the urethra. Also during this stage, a flap is cut out of the mucous membrane of the cheek, usually on the left side. The treated flap is placed on the previously prepared lower surface of the penis and fixed with multiple sutures. The wound in the oral cavity heals within a few days. After surgery, a bandage is applied to the penis and a urethral catheter is inserted. The catheter and the bandage are removed after a week. In the future, for a speedy recovery, it is necessary to treat the flap with creams and ointments. In addition, physical therapy is performed to lengthen the penis.
Stage 2 – formation of the urethra and reconstruction of the skin of the penis and scrotum. It is performed 6 months after stage 1. During the operation, the already established mucous flap is isolated, rolled up into a tube and stitched. This is how the urethra is formed. The skin of the penis is distributed evenly and naturally. In cases where there is a deficiency of the skin of the penis, skin scrotal flaps are used. After surgery, the patient remains with 2 tubes, one of which is short - the urethral stent - in the urethra, and the second is long - the cystostomy, which provides a free outflow of urine from the bladder and is located down from the navel along the midline. The presence of a cystostomy makes it possible to ensure reliable urine flow during the entire postoperative period. A soft, well-fixed bandage is applied to the penis, which is removed on the 7th day after surgery.
Hospital stay is possible for 2-3 days. In the future, the patient regularly comes to the hospital for bandages. On the 7th day after the operation, the bandage is examined and removed, and the examination is repeated 2 days later.
