A review article by Marcia L. Buck on the intranasal administration of antiepileptic drugs to children has been published in the journal Pediatric Pharmacotherapy. Vasily Nogovitsyn, a neurologist and epileptologist at the EMC Children's Clinic, Candidate of Medical Sciences, shared his comments on the article:
In modern epileptological practice, anticonvulsant administration methods that differ from traditional intramuscular and intravenous injections have long been widespread. This is due to the fact that convulsive epileptic status can create technical difficulties when trying to administer an antiepileptic drug intravenously and there is a need for an alternative route of administration.
In Europe, diazepam for rectal administration is very common, the bioavailability of this form allows the use of rectal diazepam for epileptic status, and in some countries this can be done by the parents of patients. Nevertheless, despite the advantages of rectal surgery, there are obvious social barriers to such an intervention.
Technical difficulties with epileptic status can also be encountered with the administration of buccal midazolam. Based on this, the development of intranasal systems for the administration of antiepileptic drugs is promising, taking into account the potential speed and convenience of their use even by people without special training, for example, parents of a child with epilepsy.
In Russian practice, such new forms of anticonvulsants are not widespread yet due to the lack of official registration. Nevertheless, the progress of pharmacology and epileptology allows us to hope for the future of mastering such techniques. In addition, in everyday practice, a doctor may encounter prescriptions for these forms of drugs from colleagues from other countries, and we believe that knowledge about the possibilities of helping this difficult group of patients should be consistent with current global trends.
