Surgeon

A surgeon is a doctor who works with pathologies that potentially require surgical intervention, or is engaged in reconstructive or aesthetic surgical transformation of the human body. The main sections of surgery

Surgical treatment may be required for diseases of various organs and tissues.

The main sections of surgery:

  1. General surgery. A general surgeon is a general practitioner who performs operations on the abdominal and thoracic cavities, for example, treats hernias, intestinal obstruction, and appendicitis. In addition, the doctor removes benign skin growths, including purulent ones: abscesses, boils, lipomas, fibroids, ingrown nails, etc. As a rule, it is the general surgeon who conducts the initial admission of patients, performs diagnostics, treatment of wounds and injuries, stopping bleeding, etc.
  2. Colorectal surgery. Surgical methods in coloproctology are used to treat pathologies that have not been cured by conservative therapy: hemorrhoids, fistulas, anal fissures, rectal prolapse, fecal incontinence, neoplasms, etc.
  3. Neurosurgery. A branch of medicine that deals with the surgical treatment of diseases of the nervous system. For the success of neurosurgical operations, the most modern diagnostic and treatment tools are needed, as well as the highest qualifications of doctors.   10. Plastic surgery. Plastic surgeons are engaged in the elimination of cosmetic defects of the face and body, which may be of congenital origin, or appear as a result of injuries, diseases, previous surgical treatment. Goals of surgical intervention

Depending on the medical situation and indications, surgical intervention can be performed for various purposes:

  1. Diagnostics. Surgeries are performed to clarify the stage of pathology and diagnosis, if this cannot be achieved using non-invasive diagnostic procedures. A typical example of diagnostic surgery is a biopsy if a malignant neoplasm is suspected. Through a small puncture, the surgeon inserts a biopsy needle into the patient's body, through which he selects tissue particles for cellular analysis. In the EMC clinic, this operation is performed under the supervision of ultrasound, CT and MRI.
  2. Treatment. Therapeutic surgical intervention can be performed to remove tumors, repair damaged organs or tissues, correct congenital or acquired abnormalities or defects, eliminate infections (for example, when removing appendicitis, abscess, boil, etc.). Medical operations also include transplantation, amputation and other types of surgical intervention.
  3. Relief of the patient's condition. If it is impossible to achieve a complete cure of the patient using existing methods, palliative treatment is carried out, which helps to reduce severe symptoms (for example, reduce the intensity of pain) and prolong life.

Surgical treatment methods

According to the degree of urgency, the following types of surgical operations are distinguished:

  1. Emergency. They are carried out immediately due to the threat to the patient's health and life. As an emergency, the patient is operated in case of acute appendicitis, peritonitis, acute intestinal obstruction, paraproctitis, gastrointestinal bleeding, injuries, infected wounds, etc.
  2. Urgent. They are prescribed when surgical intervention can be postponed for a short period of time to clarify the diagnosis and examination.
  3. Planned ones. Surgeries are scheduled for conditions that do not pose an immediate threat to the patient's life, so there is an opportunity to conduct a full examination and thorough preparation before surgery.

In addition, there are special types of operations:

  1. Microsurgical. They are performed under magnification of 3-40 times using a special microscope or magnifying glass, as well as microsurgical instruments.
  2. Endoscopic (laparoscopic or thoracoscopic). They are carried out through the natural openings of the body, small incisions or punctures in a closed manner, under the control of a video probe.
  3. Endovascular. They are performed on blood vessels by percutaneous access under the control of radiation imaging methods (radiography).

In what cases is it necessary to consult a surgeon

Referral to a surgeon is given by a general practitioner, pediatrician or general practitioner, or a specialist doctor who has discovered a pathology requiring surgical treatment.

A surgeon should also be consulted in the following cases:

  • soreness during movement, limited mobility, pain, swelling, redness of joints;

  • injuries to bones, joints, and soft tissues;

  • intestinal pathology (after consultation with a gastroenterologist or proctologist);

  • dermatological problems and injuries that are accompanied by inflammation or infection (boils, abscesses, ulcers, burns, wounds, bites, etc.);

  • congenital pathologies or aesthetic defects;

  • acute pains of unknown origin.

Benign disease
I have a benign lump in one breast size of 12.0*9.9 mm. Puncture or a biopsy will be done next week. I was told by mammologist that surgery is needed. As far as I know, concerning the surgery, axillary lymph nodes are to be removed together with the tumor. I also know that in Europe lymph nodes are testes for
specific markers and only affected ones should be removed; if lymph nodes are no affected, they are not to be dissected and the surgery is minimally invasive. So what is your approach? Does it make sense to do it or you have the same methods and the same equipment?

If histological examination of the sample reveals fibroadenoma of basic type or tissue hyperplasia without atypia, or nodular type fibrocystic condition of the breast tissue, the question of surgical treatment should not arise. If biopsy reveals giant fibroadenoma sectoral resection is indicated, i.e. mass excision
within the healthy tissues and lymph nodes will be removed. In case of non- benign histological result, i.e. carcinoma is detected, subsequent immunohistochemical examination is required as well as a clinical oncologist and surgeon consultation; and the decision on complex treatment will be taken by case management team. With regard to the diagnosis and treatment methods in our center, each case is addressed individually. Sometimes we remove a benign area (for example, the area of hyperplasia with atypia) using the vacuum-needle technique through 3-4 mm incision. Uterine cancer
My mom was diagnosed with the uterine cancer. She is 68 years of age and has an obesity of 4th grade (the growth of 166 cm, weight 135 kg) and hypertension. Admission to the radiology department was recommended. What should we do? As far as I know the surgery is the only method for cancer of the uterus to be removed.
Is it really so that this surgery is only possible for young and relatively healthy persons?

It is not quite so. We can operate on any patient, but the issue is which complications can lead to patient’s death and which of them can just delay the recovery. From the anesthesiologist’s point of view, it is a major challenge to intubate patients with 4th degree obesity; the abdominal section is also possible,
but there is a 100% risk of suture line disruption and inflammation, let alone the postoperative pneumonia, venous thrombosis, etc. There is another option such as vaginal hysterectomy which is more acceptable and relatively safe in obese patients. It is not a «treatment standard», however, as it allows not obtaining pelvic washings, but still there is a possibility of complete cure. Anesthesia remains a problem - both general and spinal. Radiation therapy without surgery is another acceptable treatment option besides vaginal hysterectomy. A chance of complete cure is still exists, but the survival rate is on average lower than in surgical treatment
